Certified nurse practitioner, Melanie Connell, has joined St. Peter’s Health Partners Medical Associates Home Based Primary Care Program in Troy.
Connell’s professional interests include palliative care, hospice, and ethics.
She received her master’s degree from Russell Sage College in Troy, and her Bachelor of Arts Degree from SUNY Geneseo.
Connell previously worked as a nurse at Albany Medical Center in Albany.
The SPHPMA Home Based Primary Care Program provides quality, at-home medical care for patients dealing with chronic illnesses who are unable to get to a medical office. In addition to in-home care, patients also have access to providers by phone, 24 hours a day. The program’s office is located at 433 River Street in Troy.
