Eddy SeniorCare PACE Expands to Albany and Rensselaer Counties

On January 1, 2020, Eddy SeniorCare expanded its Program of All-Inclusive Care for the Elderly (PACE) to a second location. Located in Latham, the newly renovated space at St. Joseph’s Provincial House on Watervliet-Shaker Road will enable Eddy SeniorCare to expand its PACE services into Albany and Rensselaer counties.

The expansion is the result of a previously announced agreement between Eddy SeniorCare and The Sisters of St. Joseph of Carondelet. As part of that collaborative agreement, Eddy SeniorCare will provide care for the aging sisters, who will remain at the Provincial House.

PACE is a unique health management program designed to help frail elderly adults live independently in their community, manage their medical conditions, and avoid nursing home placement. Participants in PACE receive all of their health care from Eddy SeniorCare staff and contracted network providers. The PACE program provides all Medicaid- and Medicare-covered benefits such as adult day health, physician, nursing, rehabilitation, social work, transportation, meals, personal care and coverage for medical specialists, inpatient hospitalization, and nursing home care.
