St. Peter’s Hospital and CDPHP held a joint news conference and ribbon-cutting ceremony on January 3 to officially open CDPHP’s new Customer Connect location.
Located on the first floor of St. Peter’s Hospital, right off of Main Street, the Customer Connect offices are staffed by the CDPHP Patient Care Team. The team will coordinate care, answer questions, assist with medication management, and collaborate with St. Peter’s staff on discharge planning for CDPHP inpatients. In addition, anyone is welcome to stop in to chat with a CDPHP representative and enroll in CDPHP health plans.

The site at St. Peter’s Hospital is CDPHP‘s fifth Customer Connect location in the Capital Region.
