Leaders from St. Peter’s Health Partners and Trinity Health will discuss “A New Vision for Care Coordination” at a Town Hall event held by the Innovative Health Alliance of New York (IHANY). All IHANY care team members, including physicians, nurse practitioners, physician assistants, nurses, care managers, practice managers, and staff are welcome to attend, as are hospital leadership and administration.
Topics to be discussed include:
- The Cleveland Clinic’s approach to Care Coordination
- A brief overview on how Care Transitions is critical to Value Based Care and Total Cost Management
- What “Integrated Care Coordination System (ICCS)” is being developed at St. Peter’s Health Partners
- How an ICCS will benefit IHANY members and your IHANY attributed patients.
