The HomeCare Elite reviews Medicare-certified home health agencies across the country and identifies the top 25 percent of agencies based on publicly available performance measures, including quality outcomes, best practices/process measure implementation, patient experience, quality improvement and consistency, and financial performance. The report, a market-leading review, is conducted by OCS HomeCare by National Research Corporation and DecisionHealth. In this role, Baker will oversee The Eddy’s comprehensive continuum of health care, supportive housing and community services, including seven nursing homes, five retirement campuses, two regional Alzheimer’s centers, a dedicated rehabilitation hospital, a home care division, The Community Hospice, and many other community service programs. The Eddy is a nationally recognized leader in compassionate, high-quality senior services. The network reaches 22 counties of northeastern New York and serves more than 47,500 people yearly. As the clock ticked past midnight early Saturday morning, October 1, the future of health care delivery in the Capital Region changed. The merger of Northeast Health, St. Peter’s Health Care Services and Seton Health – nearly four years in the making – became final on October 1 when St. Peter’s Health Partners (SPHP), a new not-for-profit organization that will be the “parent corporation” for the three organizations, became operational. The merger creates the region’s largest and most comprehensive not-for-profit network of high-quality, advanced medical care, primary care, rehabilitation and senior services. These state-ofthe-art services and programs will be provided through Albany Memorial Hospital and St. Peter’s Hospital in Albany; Samaritan Hospital and St. Mary’s Hospital in Troy; Sunnyview Rehabilitation Hospital in Schenectady; as well as The Eddy system of continuing care and The Community Hospice. The new organization has more than 11,700 employees at more than 125 locations in seven counties of northeastern New York.
